CLINICAL REVIEW Hypercholesterolaemia and its management

چکیده
Hypercholesterolaemia is one of the major causes of atherosclerosis. Although there are many causes, hypercholesterolaemia is the permissive factor that allows other risk factors to operate. The incidence of coronary heart disease is usually lowwhere population plasma cholesterol concentrations are low. In Britain coronary heart disease is a major cause of mortality, and a recent Department of Health survey suggested that the average plasma cholesterol concentration in the United Kingdom was 5.9 mmol/l, much higher than the 4mmol/l seen in ruralChina and Japan, where heart disease is uncommon. Many studies before and after the introduction of statins have indicated that reducing theprevalenceofhypercholesterolaemia is an important means of decreasing coronary risk.

CLINICAL REVIEW Statin induced myopathy
Since their introduction for the treatment of hypercholesterolaemia in 1987, the use of statins has grown to over 100 million prescriptions per year. About 1.5-3% of statin users in randomised controlled trials and up to 10-13% of participants enrolled in prospective clinical studies develop myalgia. As a conservative estimate, at least 1.5 million people per year will experience amuscle relate...
متن کاملImproving the cost-effectiveness equation of cascade testing for familial hypercholesterolaemia.
PURPOSE OF REVIEW Many international recommendations for the management of familial hypercholesterolaemia propose the use of cascade testing using the family mutation to unambiguously identify affected relatives. In the current economic climate DNA information is often regarded as too expensive. Here, we review the literature and suggest strategies to improve cost-effectiveness of cascade testi...
متن کاملGenetic Architecture of Familial Hypercholesterolaemia
PURPOSE OF REVIEW Familial hypercholesterolaemia (FH) is an inherited disorder of low-density lipoprotein cholesterol (LDL-C) which is characterised by a raised cholesterol level from birth and a high risk of premature coronary heart disease. In this paper, we review the genetic basis of FH and its impact on the clinical presentation. RECENT FINDINGS Mutations in any of three genes (LDLR, APO...
متن کاملHypercholesterolaemia and diabetes mellitus.
An association of essential hypercholesterolaemia and diabetes mellitus is postulated. Case records are included to illustrate hypercholesterolaemia secondary to acute diabetes mellitus, and essential hypercholesterolaemia, independent of, but accompanying diabetes mellitus. Whereas in the former the serum cholesterol returns to normal and its attendant clinical manifestations resolve when the ...
متن کاملDefining severe familial hypercholesterolaemia and the implications for clinical management: a consensus statement from the International Atherosclerosis Society Severe Familial Hypercholesterolemia Panel.
Familial hypercholesterolaemia is common in individuals who had a myocardial infarction at a young age. As many as one in 200 people could have heterozygous familial hypercholesterolaemia, and up to one in 300 000 individuals could be homozygous. The phenotypes of heterozygous and homozygous familial hypercholesterolaemia overlap considerably; the response to treatment is also heterogeneous. In...
